Effective Monday, July 15, our Bedford Family Medicine team will be back in Bedford in a new location:

If you have an in-person appointment with one of the following providers on or after Monday, July 15, 2024, the appointment will take place at 58 Hawthorne Drive in Bedford (in-person appointments prior to July 15 will take place at 100 Hitchcock Way).

Please note: Hyunouk Hong, DO, is not moving to Bedford and will continue to see patients at 100 Hitchcock Way in Manchester.
As a Family Medicine patient, you will select one of our providers to be your primary care provider (PCP). Your PCP will coordinate your care with other departments.
New patients
At your first meeting with a family medicine provider, we will perform some testing and evaluate your current condition. We'll discuss your medical history, medications, and plan of care.
We will also:
- Provide you with information about the New Hampshire Immunization Information System (NHIIS) vaccine registry
- Discuss your participation in the registry
Returning patients
If you are a returning patient, you may use myDH to communicate securely with your providers, request or change appointments, review your medical record, and more.
