Heart and Vascular Center Appointments and Referrals

The Heart and Vascular Center coordinates care through our centrally located hub at Dartmouth Hitchcock Medical Center (DHMC) in Lebanon, New Hampshire, and through regional locations in Concord, Keene, Manchester, and Nashua, New Hampshire, and Bennington, Vermont, as well as outreach locations in New Hampshire and Vermont.

New patients

If you are new to the Heart and Vascular Center and would like to schedule a consultation or get more information, contact the appropriate DHMC clinical section to be directed to the appropriate program, provider, and location:

• Lebanon Cardiac Surgery
• Lebanon Cardiovascular Medicine
• Lebanon Vascular Surgery

For pediatric patients, please contact Pediatric Cardiology at Dartmouth Health Children's.

Please note: In some cases, you must be referred to the Heart and Vascular Center by your PCP (primary care provider) or another physician, and will not be able to set up an appointment without a physician referral.

Returning patients

Office hours

Our hours are Monday through Friday, 8:00 am to 5:00 pm unless noted otherwise below:

• Cheshire Medical Center: Monday through Friday, 8:30 to 5:00 pm
• Southwestern Vermont Health Care: Monday through Friday, 8:30 am to 5:00 pm
