New patients
In most cases, you are referred to Transplantation Surgery by your nephrologist, endocrinologist, or another doctor. Once you have been referred, your first step will be to attend a new patient education class and evaluation session.
Living Donor consent form
If you are interested in becoming a living kidney donor, we require that you sign the form below, "Consent to be Evaluated as a Living Donor". Mailing the printed and signed form to our office will help expedite the donor evaluation process. The consent form is also in the Donor Information Packet, so you can also wait until you receive the packet before signing the form. Read about the process of becoming a living kidney donor.
Returning patients

For referring physicians
To refer a patient for consultation in Transplantation Surgery, contact our Physician Connection Center. You also may contact us directly at one of our office locations.
Referrals must be accompanied by the Kidney Transplant New Patient Referral form (PDF) and the Incoming Authorization Form for the patient's medical records.
