To request an appointment for evaluation in the THRIVE Clinic, please follow the three steps below.

Step 1: Request an appointment

Once we receive your information, one of our schedulers will call you to set up a visit. If you miss the call, call us back during business hours at 603-650-5261. We work with your most recent providers to ensure we have the records we need for your visit.

Step 2: Set up a myDH account

Before your first visit, make sure that you have Zoom installed and have signed up for an account with myDH.

Step 3: Request your records to be transferred to DHMC Gastroenterology

Once you have your myDH account, you may be able to share your GI records with our team through the myDH portal. If you can't share your records through myDH, we will ask you to fax them. You'll need to call your current GI office and ask them to send records to our office. We cannot accept records via email.
