Social workers in the Office of Care Management are an integral part of a patient’s team. Social workers complete thorough biopsychosocial assessments and develop care plans to support a patient’s needs while they are accessing medical care for an illness or injury.
Care management social workers work in close collaboration with the multi-disciplinary team, are advocates for patients, and support patient, families, and medical providers to develop discharge plans that will support a person’s health and wellness beyond the hospital setting.
